Your provider or clinic may invite you to the Health Myself Portal via email. You can register your account via this email invitation by following the steps below:
Step 1: Open the invitation email from your clinic or provider and click on the "Click here to register" button.
It will look something like the email below:
Step 2: Confirm your identity.
If you are not the patient, please indicate your relationship to the patient i.e whether you are their family member, a caregiver or not related to them.
If you are the patient, click the "Yes, I am" button.
Step 3: Verify your account by providing an activation code.
On this page, your clinic will provide information regarding what your secret activation code is. For example, for the patient below, their activation code is the first four digits of their Personal Health Number.
Enter your activation code and click the "Verify" button.
Step 4: Confirm your email.
If your email is different from the one displayed, you can change it. When you're happy with the email address, click the "Confirm email" button.
Step 5: Create a password.
Your password must be 8 characters long and include at least 3 of 4 of the following:
- Uppercase letter
- Lowercase letter
- A total of at least 8 characters
After typing in matching passwords in the New password and Confirmed fields, you will be able to click the "Submit" button. Click Submit!
Step 6: Create a PIN
Your PIN is used for support and account recovery. If you ever forget your password, you will be asked for your PIN in order to reset your password.
After entering your PIN, click the "Submit" button.
After reading both documents, check the box to agree to the terms, and click the "Finish Registration" button.
Congratulations! You're all set up with your Health Myself account! You'll notice that you have 1 new welcome message after completing registration. This message may contain important information from your clinic, so please give it a read!
