Once you have created the e-form and configured the General Settings you will be able to move onto setting the Form Trigger(s) for that e-form. The trigger is the scenario in which a patient will be prompted to complete the e-form within the Portal. You will be able to set multiple triggers for an individual e-form.
- After setting the General Settings you can scroll down to locate the Form Triggers tab
- From the Form Triggers tab you can add a trigger
- The different triggers include:
- Appointment Check-In - This will prompt the patient to fill out the e-form during the check-in process
- Appointment Reminder - When a patient receives an appointment reminder, they will be directed to fill out the e-form
- After Appointment Booking - After the patient has booked an appointment the prompt to complete the e-form will appear
- After appointment request approval - Once the appointment request has been approved by the clinic staff, the patient will be prompted to answer the e-form
- Before Appointment Booking - During the initial steps of booking an appointment, the e-form will be triggered and elicit a response from the patient
- Once the trigger has been dictated you will also need to select the scope of your chosen trigger. The scope pertains to what specifier the trigger will belong to (i.e. a certain patient group, a provider's schedule, what appointment type the trigger will be associated with etc...) the different scopes include:
- Schedule - Choosing the Schedule scope will associate the trigger to a particular provider's schedule. You will need to specify the schedule in question
- Appointment Type - Specifying the Appointment Type scope will associate the trigger with a provider's schedule and a specific appointment type
- Patient Group - By defining the scope as Patient Group any patient that is part of the specified group will receive the prompt to complete the e-form when triggered
- Clinic Wide - The Clinic Wide scope acts as a general scope that targets all patient users in the Portal.
- The final field you will have to set when configuring the trigger(s) of your e-form will be the Frequency Type. Note that you can leave this blank, but by setting the Frequency Type, you will be able to limit the frequency that a patient is able to fill out this e-form.
- Days - By choosing Days as the Frequency Type you can set the amount of days within the e-form will not be re-triggered for a patient who has already been prompted to complete the form.
- Total - Alternatively, by setting the Frequency Type to Total, you can control how many times in total a patient will be able to fill out an individual e-form.
- Once these configurations for your e-form trigger(s) have been completed, you can continue to configure the e-form by using the Form Editor.
